Friday night registration is mandatory for all teams.

REQUIRED REGISTRATION MATERIALS:
USYS Registered Teams US Club Soccer Registered Teams
   
 1) Official state approved roster, plus 2 copies  1) Official US Club roster, plus 2 copies
 2) Validated USYS player passes  2) Valid US Club player passes
 3) Medical release forms  3) US Club R002 Medical Treatment forms
 4) Permission to travel - out of state teams
 		  4) Guest players - (max of 5)
      - must have valid US Club player pass
 5) Guest Players - (max of 5)
- must have valid USYS player pass, medical release form, and permission to travel from your state.
